1. Overview
The Scorecard Lead form will likely play a key role in your Scorecard flow, in particular when it comes to asking your visitors to submit their identifying information such as Name and Email. Once a visitor submits your lead form you officially have a new lead.
Each Scorecard technically shares one lead form setup, however it can appear and be used in various different ways throughout the Scorecard. Or it can be removed completely if needed.
Typically you will want your Lead form to appear either before leads progress to your main quiz questions, or just after they complete the questions but before they see their personalized results.
By default Lead forms contain some system fields such as First Name, Last Name and Email fields which can be configured but you also have the option to add any additional custom fields or questions you require.
The Lead form is also where you collect the all important opt in and consent information to comply with applicable laws such as GDPR.
Once you have configured your lead form you're able to include it into your Scorecard flow in various ways. For example within the landing page builder you will find many form sections which allow you to include the form directly on a landing page, or banner sections with buttons which can pop up the lead form in a modal. If you'd like your lead form to appear after your main quiz questions then this can be configured as an automatic pop up modal on your results page.
2. Lead Form Settings Page
You can access the lead form settings by clicking on the Scorecard you'd like to edit from your account home, and then Settings > Lead Form.
3. Adding and editing custom Lead form fields / questions
Follow the step below to add custom fields to your lead form:
1) Clicking the Add Field button under the form fields section on the Lead Form settings page. This will open a pop up window with the available options.
2) Select the Type of field you would like to add. More information on each field type available is detailed below
3) Add a Question Label, this will be shown next to the field or question on your lead form. For example Phone
4) Choose whether the field is required or not. If required your visitors will not be able to complete the lead form without inputting a valid value
5) Depending on the field type, there may be some other settings, these are detailed below
6) Each field shows a More settings button which reveals the ability to set a field key. This is used if you want to pre-populate data using query params. This is an advanced use case and rarely required. Find out more here
6) Finally click save.
You can then edit or re-order the field once back in the list view.
4. Custom field types

5. Edit the system fields (First name, Last name, Email)
The system fields for First name, last name and email behave slightly different to custom fields added the lead form. Technically these fields cannot be changed, removed or re-ordered however you can change their label or disable them. A common use case for this might be to disable the Last name field if you're happy to just take First name and email. Or you might want to change the label for Email to say something like Business Email. We do not recommend changing the labels of these fields to something with a different meaning. For example if you changed the email label to be Phone, whilst this would appear as Phone to your leads, it would still be treated as an email field and therefore wouldn't allow phone numbers to be entered. Further to this, this email field is used throughout the platform in areas such as the leads list, and in integrations so it's important it remains used as an email field.
6. Data protection settings
This is where you can control how the opt in field works on your Lead form along with an easy way to add a privacy statement and a link to a privacy policy which will be shown by the submit button.
For the opt in field your available options are:
1) Implied Consent - Your visitors will not see any optin checkbox. Within your dashboard the lead optin status will show orange with implied consent and the timestamp.
2) Explicit Consent (Optional) - Your visitors will see an optin checkbox but will be able to continue to submit the lead form without opting in. When this option is selected you will see an additional text field which allows you to customise the opt in wording. By default this is set to Opt in to receive updates via email.
When a lead ticks the checkbox and submits the lead form, a timestamp is stored along with the specific opt in wording and this will appear with a green dot on the lead page within your dashboard.
If a lead submits the lead form without ticking the opt in box, you will see a red circle on the lead page with in your dashboard.
3) Explicit Consent (Required) - Your visitors will see an optin checkbox and will be not able to continue to submit the lead form without opting in. When this option is selected you will see an additional text field which allows you to customise the opt in wording. By default this is set to Opt in to receive updates via email.

Privacy Statement and Privacy Policy URL
You can use these fields to conveniently add a small statement and a link to a privacy policy near the Lead form submit button. If you populate a URL into the Privacy Policy URL field, a simple link will appear labelled "Privacy Policy" which will open in a new tab if clicked.
7. Form Behaviour
Here you're able to configure whether you require the lead form to be completed before the quiz questions or not.
You can toggle between the following 2 options:
1) Before Questions - People will not be able to start your main quiz questions without completing the lead form first. This is enforced so if a user were to try to visit your questions directly they would be redirected back to your default landing page.
2) After Questions - People will be able to answer your questions without completing the lead form first. This setting allows you to remove the lead form from your landing page which in turn would make the lead form pop up on the result page by default although this behaviour can be customised.
Note - With this setting enabled, it’s possible for people to start your questions directly via the /questions route for example https://yourscorecard.com/questions

8. Advanced email validation
Available on the Business Tier and upwards, you can use advanced email validation to control which type of emails are allowed to submit your lead form and continue to the questions or their results. With advanced email validation enabled ScoreApp will use a more advanced system for checking the format of emails addresses and you're able to configure blocking disposable email addresses (e.g., from Mailinator, Yopmail), check the deliverability of email addresses and control the acceptance of free email providers (e.g., Gmail, Yahoo).
